$100 million baseline for every state
Each state received the same $100 million, $5 billion in all, regardless of size or rural population.
CMS awarded $10 billion for Year 1 of the $50 billion Rural Health Transformation Program, split across all 50 states. Awards range from $281.3 million in Texas to $147.3 million in New Jersey. Each state runs its own program, with its own initiatives, eligibility rules and application process.
Spending data in the RHTP Year One Report covers December 29, 2025 to July 31, 2026, the latest CMS reporting window.
States are listed A to Z, with each state's rank by Year 1 award. Open a state guide for its initiatives, eligibility and how to apply, or its section of the Year One Report for spending data through July 31, 2026.
Awards are CMS Year 1 (Budget Period 1) amounts, rounded to the nearest $0.1 million.
Half of the $10 billion was split evenly. The other half followed each state's application.
Each state received the same $100 million, $5 billion in all, regardless of size or rural population.
The other $5 billion was distributed based on each state's rural needs, the policy actions it committed to and the strength of its plan.
CMS plans up to $40 billion more through 2030 and rescores every state each year on progress, data and policy commitments.
Funds not obligated by the end of a budget period are redistributed to other states.
Year 1 awards range from $281.3 million for Texas to $147.3 million for New Jersey, $10 billion in all. Half of states received more than $201 million. Every state got $100 million in baseline funding, and the rest of each award was based on the state's rural needs, its policy commitments and the strength of its plan.
Each state runs its own program through a lead state agency named in its application, such as the Ohio Department of Health. The agency manages the plan, posts funding opportunities and answers questions. State guides on this page name the agency, link to its RHTP page and list how to reach it.
Apply through your state, not CMS. States award grants and contracts through their own solicitations and procurement sites, and each round states who is eligible. Start with your state's guide or lead agency, and watch for new rounds after CMS approves each budget.
The RHTP Year One Report tracks how much of each state's award was put to work through July 31, 2026, by initiative and recipient, using state-reported data from CMS. It updates after each CMS data release.
